Rental Assistance

You should be working to satisfy ALL of your financial obligations.

Please provide copies of:
Valid picture ID (For household members over the age of 18)
SS cards and/or birth Certificates (For all household members)
Annual Income amount
Proof of income (pay stubs and/or unemployment documentation)
Copy of lease or mortgage statement
Copy of your court eviction document (if applicable)
If you are seeking for rental assistance, your landlord/property manager must sign off the amount of rent that is due.
If you are seeking utility assistance please provide your most recent utility bill. The address, amount due, and account number must be visible
